Notice of Appeal Form

To file your appeal, complete all sections of the Notice of Appeal form and submit it to the Community Care and Assisted Living Appeal Board. Options for how to submit your form are listed at the bottom of this page.

The Notice of Appeal must be delivered within 30 days of you being notified of the decision that you want to appeal. If the appeal will be filed later than 30 days, you must apply to the Board to extend the time to file the appeal (see Schedule 1 on last page of the form).

The Board has created the Notice of Appeal form linked above and recommends you use this form because it provides space to fill in all the information you must provide to start an appeal. If you choose not to use the form linked above, you must instead ensure that a written notice of appeal is filed with the Board, and it must include all the following:

  • identify what decision is being appealed,
  • state why the decision should be changed,
  • state the outcome requested,
  • contain the name, address and telephone number of the person or organization who is appealing the decision,
  • if someone is acting on behalf of the person or organization who is appealing the decision, then also include their name and telephone number,
  • an email address for delivery of any notices and correspondence about the appeal,
  • be signed by you (the appellant), or by the person (a lawyer or an agent) who is acting on behalf of the appellant.

When you file your notice of appeal with the Board, you must include a copy of the decision you are appealing. At the same time, you must deliver a copy of the notice of appeal, and a copy of the decision you are appealing, to the respondent (who is the person who signed the decision that you are appealing).

There is no fee for filing an appeal.

The Board will send a letter acknowledging the notice of appeal and will provide you with more information about the appeal process.

Options for Submitting Your Notice of Appeal to the Board

To file a notice of appeal, Appellants may email, courier, hand deliver, or mail the Notice of Appeal to the address on the Board’s home page. The preferred method for filing a notice of appeal is by email.
